Daily Symptom Checker During Shut Down and Winter Break

Employees who will be working either remotely or on campus must complete the Daily Symptom Checker.

Daily Symptom Checker (DSC)

Faculty are not required to complete the DSC when they are not on contract.

Employees are not required to complete the DSC when they are on vacation, holiday, sick, or on leave.

Supervisors should give waivers to employees on vacation, holiday, sick, or on leave so they will not receive the automated system notification to complete the DSC. Please watch this video for a tutorial on how to do this.

Employees who will be working either remotely or on campus must continue to complete the DSC each day they are performing work. The first question of the DSC will ask employees if they are physically on campus that day. If they answer no, they will be directed to answer one additional question for that particular day. If the employee is physically working on campus that day, they must complete the entire form.

Human Resources (HR) will continue to monitor this daily and will contact any employee who answers “yes” to any of the subsequent questions after indicating they will be on campus. HR will provide guidance to the employee and follow up with the supervisor. As a reminder, anyone who answers “yes” to any of the subsequent questions must not report to campus until they speak with an HR representative.
