2023 Special Option Transfer Period for CSEA & M/C Health Insurance Enrollees

This announcement is for CSEA and M/C health insurance enrollees only. CSEA and M/C health insurance enrollees may change their plan option and/or plan coverage from June 1, 2023 through June 30, 2023.

Changes effective July 1, 2023 can be found in the Empire Plan Special Report mailed to home addresses and include reduced in-network maximum out of pocket limits, single visit copayments, Center of Excellence for Substance Use Disorder and new reimbursement methodology for non-network claims.

Employees who wish to request a change during the 2023 Special Option Transfer Period must complete and submit a Health Insurance Transaction Form (PS-404) starting June 1 and no later than June 30,2023.

Changes to the plan option include:

  • The Empire Plan to a NYSHIP HMO
  • NYSHIP HMO to The Empire Plan

Changes to the plan coverage include:

  • Change from Family to Individual coverage
  • Remove dependents from Family coverage
  • Cancel Individual or Family coverage

Certain changes to NYSHIP coverage may require a late enrollment waiting period:

  • New enrollment in NYSHIP coverage
  • Change from Individual to Family coverage
  • Add dependent(s) to existing Family coverage

Reduction in Empire Plan biweekly rates:

Salary grade 9 and below:

  • Individual plan: $54.51
  • Family plan: $235.13

Salary grade 10 and above:

  • Individual plan: $72.68
  • Family plan: $280.06
