New Electronic Registration & Record Forms
For convenience, students and faculty can take advantage of our many electronic forms. A complete list of student forms may be found on the Registration and Records website.
Registration and Records would like to welcome our students and faculty back to campus. We want to remind students and faculty we have updated many of our Registration and Records forms and they are now located online.
Permission slips are no longer used to obtain permission to add a class. With the new process, students that need permission should email their request directly to the instructor and the instructor can enter the override, which will allow students to register themselves.
Please utilize our faculty MachForms located in Secure Documents of Web Banner. The electronic forms available include:
- Incomplete contracts and contract removals
- Course Substitution and Waiver
- Grade Change
- Independent and Directed Study application
- Schedule of Classes Adjustment form
- several other registration forms designed for faculty.
We also have included video instructions on how to fill out several of the forms.
